Otitis Externa Aguda Difusa. La Otitis Externa Aguda Difusa es uno de los problemas más comúnmente tratados por el especialista en Otorrinolaringología en la práctica médica.

Concepto

La Otitis Externa Aguda Difusa es un estado inflamatorio en el conducto auditivo externo que afecta además a la capa epidérmica de la membrana timpánica.

Etiopatogenia

El comienzo de la infección coincide con la ruptura de los mecanismos de protección del oído externo producto de la limpieza excesiva del canal auditivo externo con, hisopillos, uñas, horquillas, etc., también por el lavado reiterado con agua lo cual disuelve los elementos hidrosolubles favoreciendo la proliferación bacteriana. Los lavados con agua jabonosa que crean una película alcalina en las paredes del conducto, además del contacto de este con agua contaminada. Propician su aparición, la anemia, la desnutrición, los déficits vitamínicos, la Diabetes Mellitus y la Dermatitis Seborreica. El germen predominante es la pseudomona aeruginosa pero pueden encontrarse otros como: proteus mirabilis, staphilococus aureus y staphilococus albis.

Características Clínicas

Tiene como síntomas fundamentales dolor y secreción por el canal auditivo externo, sensibilidad a los movimientos del trago y la oreja, hipoacusia, sensación de plenitud aural y en casos complicados fiebre y celulitis por contigüidad en la concha auricular.

En el examen físico se observa el conducto auditivo externo rojo, edematoso, doloroso a la tracción del pabellón auricular, piel edematosa, macerada, enrojecida, secreciones purulentas de color variable, generalmente turbia o grumosa y como signo clásico la ausencia de cerumen.


Prevención

El cerumen repele el agua, evita la maceración de la piel y su infección e inhibe el crecimiento bacteriano. Su eliminación con excesiva limpieza, lavados reiterados y automanipulación para extraerlo, deja al conducto auditivo externo desprovisto de este elemento protector importante, facilitando la aparición de la enfermedad.
